How to Apply?

1. Be sure that you have met all of the eligibility requirements (page 2 of the "Guidelines").
   
   
2. Complete the application form (2 pages).  Place it and the other required documents (3 letters of recommendation, transcripts, and essay) in a file folder or envelope.
