Form/Evaluations To be able to view some of our forms, you must have the free Acrobat Reader .  If you don't have the Acrobat Reader, get your free copy by clicking here.   Follow Adobe's instructions to download and install the software.   Once installed, the software will automatically launch when viewing a PDF document.  Participant Forms (forms completed by participants/potential participants) SSS Application Packet -- Applications to the Student Support Services are accepted throughout the year. In order to apply, you must print the Student Support Services Application and mail it to the address published on the application. Please consult the Student Support Services brochure regarding qualifications for admission. SSS Scholarship Application -- To be completed by SSS participants who need financial assistance.  Tutorial Request Application -- Need a tutor?  Complete this application to request tutorial assistance. Student Mentor Request -- Would you like a student mentor?  Complete this application to request one. Program Evaluation -- Form allows participants to evaluate the SSS program. SSS Student Advisory Committee Application -- Completed by SSS participants who would like to have input into the SSS program, its events and offerings. SSS Exit Interview  -- Completed by SSS participants who are leaving (or have recently left) Calhoun. Request for Letters of Recommendation -- Completed by SSS participants that may need Letters of Recommendation Tutor Forms  (forms completed by tutors/potential tutors) Tutor Application -- Application to become a tutor (see tutor page for more information). Employee Forms (forms to be completed by faculty and staff) SSS Participant End-of-Semester Evaluation -- To be completed by instructors on each SSS participant in their classes at the end of each semester . Campus Mentor Application Form -- To be completed by Calhoun employees that would like to become Mentors. Mentor Log-In Form  -- Completed monthly by Mentors Community Partners Forms (forms to be completed by interested members of the Calhoun community) Community Partners/Mentors Application Form -- To be completed by interested members of the community that would like to become Mentors.
