To download the registration packet, you will need the Acrobat Reader. If you don't have the Acrobat Reader, get your free copy by clicking here. Follow Adobe's instructions to download and install the software. Once installed, the software will automatically launch when viewing a PDF document.

- Camper Health form
- Authorization for Medical Treatment form

Registration Packet
Send completed registration packet with full payment to the address below. Please call Dr. Mitchell to reserve your spot. Reservations are on a first come, first serve basis, with 18 spots available for each week of Robotics camp and 15 spots available for each week of the 3D camps.
